Our History


In 1963, fifteen local people had a vision, they formed a corporation to be called Sargent Manor which was to be a 40-bed retirement home. Guests would need very minimal assistance. On November 8, ground was broken, and we were on our way. Completion of the new home was on May 28, 1964.

In June of 1969, there was yet another groundbreaking. This would be the south wing, what we now call C-wing. If was built by Rathert Construction. With this construction, we gained another 22 beds for retirement people, bringing the total capacity to 62. Also added were a tub room, boiler room and sitting lounge.

In 1973, rules for retirement centers changed. With the amount of care required for the guests, we needed to upgrade the level of care. Sargent Manor applied for a license for Intermediate Care (ICF), with the promise to meet the requirements and bring the facility up to standards. A license was issued for January 1, 1974. This would require a licensed Nursing Home Administrator, and around the clock nursing hours, etc.

DeDe Diegel was our first licensed Administrator and developed eight departments which were Activities, Business, Maintenance, Dietary, Housekeeping, Social Services, Nursing and Administration. Facility space was limited. Dining was done is shifts and the State was requiring activity space.

In June of 1977, another remodeling project was started. This included a new dining room, kitchen, pantry, storeroom, Administrator and Director of Nursing office. In 1978, we were happy to move into the new quarters. The dining room held 62 residents. A new very modern kitchen was appreciated, and we had air conditioning through the main portion of the facility.

In 1982, a new ambulance entrance and single stall garage was built on the west side of C-wing. This worked wonderful when loading and transferring residents. We also had a place for our Bobcat.

Having been locally owned by shareholders for almost 20 years, the manor was sold to HMU Associates, Edina, Minnesota. Glen Urquhart was our new owner. The purchase became final on July 1, 1983. We were the first nursing home managed by HMU and became the flagship. The chain grew to 10 nursing homes before the company split. HMU owned two other nursing homes in North Dakota; one in Cando and one in McVille.

There was more remodeling done in the late 90’s. A new laundry was built where the ambulance garage was which gave us a new and bigger OT/PT room was completed, and a freestanding garage was built.

In the year 2000, the facility was downsized to a 40-bed nursing home because of very low occupancy.

The Forman Community Development Corporation had been trying to purchase the Sargent Manor and finally in March of 2001, it became finalized and we were The Four Seasons Healthcare Center, Inc. A 501c3 not for profit status was awarded.

In 2010 the facility once again down-sized and became a remodeled 37-bed skilled facility. Then in 2014 the Board of Trustees of FSHCC decided that the Forman community would benefit from some type of non-skilled elderly care and thus construction of the Four Seasons Villas Assisted Living Center began. The Four Seasons Villas is a 12-apartment complex offering services to those individuals who need minimal support and care.

In 2018 the Board of Trustees of Four Seasons Healthcare Center decided that the nursing home needed to offer more private rooms and construction began on an 8-room addition to the north side of the existing building. In addition to 8 more private rooms this addition will offer a new PT/OT room, a staff training room, another T.V. lounge, a volunteer room and a Chapel. We are anxiously awaiting final approval from the State of ND with a tentative move in date of June 10, 2019.
