The Four Seasons Healthcare Center is a 37-bed skilled nursing home that has provided care for the elderly in the community for the last 61 years. We have 8 suites in the north addition that was built in 2018. We have private, and semi-private rooms that accommodate Medicare and Medicaid residents.

We are staffed 24 hours/7 days a week with Nurse on each shift and CNA’S.

Physical Therapy, Occupational Therapy, Speech Therapy, and Respiratory Therapy services are available up to 7 days a week if needed in the facility. We have a full time Restorative Program that is run by a Certified Restorative Aide.

We have an Activity Program that runs from 9am-5pm, with many exciting activities planned around what our residents love to do. We do have activities that run outside these times as we go on outings to baseball games, shopping trips, fishing trips, zoo trips, and Storybook Land.

We have open breakfast from 7am to 9am every morning, regularly scheduled noon, and supper meals with afternoon/evening snacks available to residents. Residents can have a dorm fridge in their rooms for food and drinks as well. A community fridge is available for residents to put food in as well that can be accessed 24/7.

Housekeeping/Laundry is completed 7 days a week during the day.

Maintenance takes care of our building everyday outside and inside needs as well as the items that need to be fixed that staff, residents, and families request assistance with during the day.

Transportation is provided by trained staff to appointments and outings. We currently have two vans available for transporting residents to accommodate multiple appointments on any given day.

We have a Chapel in the new addition for Church services for our residents to safely utilize with Covid precautions being able to be 6 feet apart.

COVID-19 Vaccinations: We have a 100% vaccination rate of our Residents and 94% vaccination rate of our Staff currently for Covid-19.
